By signing below you agree that:

• Your dog is healthy and has not undergone antibiotic treatment in the past 30 days. The first time your dog stays with us, you will bring a current health certificate from your vet when you drop your dog off. We will request copies of your dog's annual exam to keep on file.
• Your vet shall release all medical information to us pertaining to your dog's health should we feel it necessary to obtain. If you decide to entrust your dog to us, we will notify your vet that we will be caring for your dog and authorized to make decisions on your behalf. (we will make every reasonable attempt to contact you if an emergency should arise and your dog require veterinary attention).
• Your dog has never bitten or injured another dog or human. Should your dog injure another dog or human or expose another dog to illness while in our care, you will be financially responsible for any and all vet/medical bills incurred for treatment of such injury/illness. You must leave a valid credit card number on file with us should we need to take your dog for treatment.
• Your dog has not recently (within 14 days) been treated with Frontline or any other flea/tick product.
• Your dog has NOT been vaccinated in the last 14 days (we don't allow freshly vaccinated dogs to stay due to the undesirable vaccine shed the other dogs will be exposed to)
• Your dog is under 30 pounds
• Your dog is spayed/neutered*, free of contageous disease, friendly with other dogs, house-trained, does not suffer seperation disorders or other serious training issues like excessive barking or marking territory and is not food or toy aggressive. NICE DOGS ONLY!

*we do accept unspayed/neutered dogs if they are under 6 months old or an active show dog.
